Fresh Passport 
(Documents Required)  All originals + 2 sets of photocopies each

• Stay Proof : Ration card / Rent receipt / Electricity bill /Bank passbook /Society letter / Company letter (Ltd.Co.) / Voter's Identity card. (For minimum one year stay proof).

• Date and Place of Birth Proof: Birth Certificate (issued by a Municipal Authority), School or College leaving Certificate, S.S.C Board Certificate.

• Photographs: 10 Photos (Frontal view passport size photo with plain background)

• Qualification Proof: Degree Certificates , 1st, 2nd, 3rd Years Mark sheets, Passing certificate.

• TAX PAYEE: Form 16.(Salary deduction Certificate for 3 years, & Tax Return Challans) 3 Yrs.Self- Assessment Tax paid challans, Assessment orders and Appendix "A" tax clearance certificates (with PAN card) .

• No Objection Certificate: Required in case of Employees of Govt. / Public Sector / Statutory body.

• Marriage Proof: Marriage certificate issued by a "Registrar of Marriage", Or Marriage photo, Invitation card,& Marriage Affidavit as per format GIVEN BY Passport Office and 
  also assisted by Metropolitan magistrate only (Notary not allowed).

• Time Taken:
  Approx. Two and Half Months.
  Depending upon C.I.D Verification from local police station & SB II C. I.D.

• CHARGES: Rs 1500/- (Including Passport office charges). Applicant has to clear the police report from his nearest police station within 1 month from the date of submission& for the same he should carry all original documents, 2 photos and 2 sets of Xerox copies which you have submitted to the passport office.

• Affidavit: Minor Affidavit as per Passport Office format

• Time Taken: Approx. One and Half Months

• For apply fresh passport, new passport form to be filled in capital letter with full initial with photographs should be pasted on appropriate place & sign by Legal Guardian across the photo Half on the form & Half on the photo but it should not go on the face. (Only Page No-4 within two box Childs sign required).
